Infantile Haemangioma

Benign  vascular  tumor  which  appears  at  birth  and  continues  growing  till  9  months  then  starts  involuting. 

Clinical presentation 

  • Red macule at birth
  • Grows progressively
  • regresses from 9-12 months
  • May be part of PHACES syndrome; Posterior fossa malformation, haemangiomas (mainly facial), arterial,  cardiac, eye, and sternal anomalies 

Indications for treatment 

  • If interferes with feeding
  • If interferes with breathing
  • If interferes with vision

Investigations 

  • FBG before initiating treatment and serially after
  • ECG
  • Record heart rate and blood pressure before treatment

Pharmacological Treatment 

A: propranolol (PO) 1mg/kg divided into 3doses given 8hourly in 24hours. 

Monitor heart rate and blood pressure 2hourly for 3days if tolerated 

Increase the above-mentioned medication to 2mg/kg/day in 3divided doses till lesion regresses  Adjust with change in weight.
